There are 3 types of question formats available. |

Clinic Surveys online has provided for 3 types of questions -1. standard 5 option from bad (poor) to good (positive) - 2. two sets of 5 options as above but these are one set for a person sense of importance and a set for their response - 3. Free form text response. The summary reporting handles these 3 type differently . |
